REPORT GRANT DECISIONS
Decisions are due April 15, 2009.

A complete Roundtable Decision Report includes:(Grants will not be processed without a Penny Harvest Evaluation from the Gather Pennies Phase- available for download below).

FORM A: Process Report

·      School overview – 1 per school

·      Gather Pennies – 1 per school

·      Make Grants/Take Action/Party&Plan – 1 per roundtable

 

FORM B: Grant Decision Report — 1 for each grant decision

·      Remember to include the organization’s information, including their 501c3 number.

·      Complete a Form B for all service projects, even if they didn’t cost any money.

FORM C: Share Your Story

 Bring one of your grants or service projects to life. Share the story behind the decision.

FORM D: Penny Harvest 09/10 Enrollment — 1 per school

 

Penny Harvest Report

Remember to download and complete the Penny Harvest Evaluation Report: Gather Pennies.  Reports were due January 7, 2009. 

These reports provide us with the much needed feedback from the very person implementing our program- you!
